The INSTITUTE FOR MULTICULTURAL COUNSELING AND EDUCATIONAL SERVICES (IMCES) is a non-profit community clinic established in 1989. IMCES has been operating based on mission and value-driven and outcome-based service delivery to the community since its inception. IMCES provides a spectrum of comprehensive mental health services to underserved/ under-resourced individuals with mental health challenges. Services are designed for children, youth, and adults.
We are looking for an experienced Case Manager for the position of Program Coordinator/ Program Manager.
The Case Manager must have a proven record of experience and skillset in many domains of case management, including some specialty areas such as housing, employment/ job development, rehab, addiction/ substance use, legal, and other resource development.
In this role, you will be responsible for managing the day-to-day operations of the FSP Program, ensuring that clients receive high-quality, individualized care and support.
This position is intertwined with direct service to clients, carrying a caseload.Key Responsibilities:
- Ability to manage a team of case managers with different specialty areas with the utmost collaboration, coordination, and leadership skill set to facilitate the best outcome services to clients.
- Oversee new clients' intake process, ensuring they are promptly connected with appropriate services and resources.
- Develop and implement individualized treatment plans in collaboration with clients, their families, and other treatment team members.
- Monitor client progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Coordinate care with other service providers, including mental health professionals, medical professionals, housing agencies, and social service organizations.
- Ensure compliance with all relevant regulations and guidelines, including those set forth by regulatory agencies and funding sources.
- Collect and maintain accurate records of client assessments, treatment plans, progress notes, and other documentation.
- Participate in program evaluation and quality improvement activities to enhance the effectiveness of the FSP Program.
- Provide crisis intervention and support to clients as needed, including afterhours support on a rotating basis.
- Stay informed about developments in the field of mental health and incorporate best practices into program operations.
Qualifications:
- Minimum of 3 years of experience in the mental health field, with a focus on serving individuals with severe mental illness.
- Familiarity with online health records systems (e.g., EXYM).
- Previous supervisory experience is strongly preferred.
- A strong work ethic, organizational skills, and time management skills.
- Excellent communication skills, both verbal and written.
- Good understanding of the dynamic of the severely mentally ill (SMI) target population and needs for relevant interventions.
- Ability to work effectively as part of a multidisciplinary team.
- Commitment to cultural competence and excellence with sensitivity in working with diverse populations.
