    The JobThe purpose of the Bereavement Behavioral Health Specialist is to provide support to members coping with the loss of a family member and/or loved one.

    The main goal is to give bereavement support, guidance, and clinical assistance, helping prevent negative impacts of loss and facilitating the transition into a primary caregiver or self-caretaker role.

    You WillThe Bereavement Behavioral Health Specialist will function in the following ways:

    Assess members to identify medical, physical, and psychosocial needs in order to prevent utilization and support members overall goals of care.

    Support members coping with the loss of a family member and/or loved one.
    Provide bereavement support, coordination of care activities and clinical assistance.

    Ensure and evaluate clinical appropriateness of care plan by incorporating assessment findings against evidence-based guidelines, clinical reasoning, clinical practice guidelines and/or best practices in the community.

    Provide motivational interviewing and engagement to connect our members to the right services through education and coaching.
    Help our members get access to care and bridge them to supportive therapy.
    Create interventions and overall plan of care for members dealing with grief with a potential for BH/MH diagnosis.
    Strong collaboration with external providers, specifically with BH/MH vendors and programs.
    Enhance department and organization activities by identifying opportunities for improvement to existing processes.

    Participate in activities to support professional growth through education opportunities, reading professional publications, maintain professional networks, and participating in professional organizations.

    Comply with all regulatory and quality agency standards including:

    Centers for Medicare and Medicaid Services (CMS), Department of Managed Health Care (DMHC), Department of Health Services (DHS), and accreditation bodies standards such as the National Commission of Quality Assurance (NCQA) as it relates to care management activities.

    Actively support the achievement of SCANs vision, mission, and goals.
    Other duties as assigned.
    Your QualificationsMaster's Degree in Social Worker required. LCSW required.2-5 years Behavioral and/or Mental Health Case Management experience, required.
    Hospice and/or Bereavement experience, strongly preferred.
    Strong knowledge & work experience with coaching & motivational interviewing with the ability to build strong rapport.
