Responsibilities:

Conduct comprehensive nutritional assessments for residents and develop individualized care plans based on their dietary needs, preferences, and medical conditions.
Collaborate with interdisciplinary team members, including physicians, nurses, and therapists, to ensure coordinated and effective care delivery.
Monitor residents' nutritional status and make appropriate adjustments to their diet plans as needed.
Provide nutrition education and counseling to residents and their families, promoting healthy eating habits and lifestyle choices.
Maintain accurate documentation of nutritional assessments, interventions, and outcomes in residents' medical records.

Requirements:

Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution.
Current registration as a Dietitian by the Commission on Dietetic Registration (CDR).
State licensure or eligibility for licensure as a Dietitian/Nutritionist.
Strong communication and interpersonal skills, with the ability to effectively collaborate with diverse team members and interact with residents and their families.
Compassionate and patientcentered approach to care, with a commitment to promoting residents' overall health and wellbeing.
Prior experience in a longterm care or clinical setting preferred, but not required.
