This professional social service position is assigned to the Early Intervention Services Unit of the Lycoming-Clinton MH/ID Program.

They need to demonstrate knowledge of and understanding about the following:


  • Infants and toddlers with disabilities
  • Part C of the Individuals with Disabilities Education Act and accompanying federal regulations
  • Early Intervention Services System Act and accompanying state regulations
  • The nature and scope of Early Intervention services available under Chapter 4226 Early Intervention Services and the funding available.
This position will be under the supervision of the Early Intervention Supervisor.


ESSENTIAL FUNCTIONS:


  • An individual must be able to successfully perform the essential functions of this position with or without a reasonable accommodation._
- *Coordinate and prepare all referrals from Physicians, Headstart, Parents. _

  • Send for prescription for Early Intervention Services _
  • Coordinate and work with Providers on enrollment and maintaining Provider Registry information. Ongoing support to Providers for entering training hours. _
  • Working closely with EI supervisor to address Documentation missing from Docushare and Provider Registry. Assist in implementing Policies/Strategies, to address any concerns or missing information. _
- *Assist parents with enrolling in Medical Access Coverage if child qualifies for Early Intervention Services. _

  • Assist with getting Family Survey out to families. _
  • Assist with Provider Monitering/ Observations and promoting the Coaching Model_
- *Participate in Core Leadership Team to implement and measure how the Coaching Model is being implemented within the Early Intervention program, by reviewing mínimally 10 records and Provider Session Notes, per quarter and provide feedback to Providers._


PHYSICAL ACTIVITIES:


  • The following physical activities are necessary to perform one or more of the essential functions of this position._
  • Communicate effectively in person or by using telecommunications equipment.
  • Input and gather data on a computer.
  • Proofread documents accurately.
  • Understand oral and written information
  • Move and relocate work related items and, if applicable, assist clients with personal items without assistance.
  • Move and/or relocate child(ren) without assistance, if applicable.
  • Enter, exit and move about multiple residential and office settings
  • Travel to client's homes/residences in order to serve and meet their needs in their own environments as appropriate.
  • Operate standard office machines

Pay:
From $47,548.80 per year
