Skills / Requirements:
This position is responsible for the coordination of all systems/services required for an organized, multidisciplinary team approach and assures quality, cost effective care for the identified patient population.
Minimum Qualifications
Education/Specialized Training/Licensure:
Licensed Master Social Worker (LMSW) required
Licensed Clinical Social Worker (LCSW) preferred
Related specialty certification required within two years of employment
Holds a current licensure in the State of Texas
Work Experience (Years and Area): 2 years Medical Social Work
