- Conduct a thorough case management assessment covering key life areas such as housing, finances, transportation, legal aid, employment services, healthcare, and family dynamics;
- Deliver case management services to ensure individuals are well-informed and able to access various medical and support services, like legal aid, financial guidance, educational opportunities, and employment counseling;
- Create a person-centered Integrated Family Case Plan with recovery strategies, priorities, desired outcomes, and necessary resources;
- Provide training to clients on identifying and managing relapse triggers;
- Connect individuals with relevant community resources, including specialized services for HIV/AIDS, mental health issues, and health problems linked to substance abuse or legal matters;
- Offer education on HIV and Hepatitis;
- Support individuals in reaching their maximum recovery potential by assessing and developing their skills, offering educational opportunities, job readiness training, and career development;
- Ensure regular documented visits and services to individuals based on their needs and preferences;
- Utilize engagement techniques to motivate individuals to access beneficial services for a healthy lifestyle;
- Refer and assist clients in accessing substance use disorder treatment;
- Aid individuals in transitioning to the next level of care if required;
- Coordinate interventions and services across multiple agencies;

We are looking for a STAR Case Manager to be a part of an addiction-centered program situated in Burlington County.
The STAR Program extends assistance to adults facing challenges in Burlington and Cumberland Counties, commonly linked with Opioid Use Disorder (OUD). These challenges include homelessness, incarceration, legal matters, employment, education, transportation, need for social services, healthcare, child welfare involvement, child care, health insurance, and treatment options like Medication Assisted Treatment (MAT).
Our primary objectives are to help individuals sustain their progress in the Recovery Zone, minimize the risk of recurring opioid-related issues, and prevent future overdoses. The STAR teams are reachable by phone beyond typical work hours, ensuring round-the-clock coverage, and offering services for up to 12 months.
Schedule: Monday to Friday with varied shifts as per program requirements and two evenings per week.
