Job Summary and Qualifications:


As a Medical Social Worker, you will be responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.

You will work as a liasion between the patient and the community resources/post-acute care providers.

You will create and facilitate timely implementation of discharge plans and arrange post-acute interventions identified in the plan of care.

You will facilitate patient movement across the continuum of care and identify, address, and track barriers to patient throughput.

  • You will respond timely to referrals from the Registered Nurse Case Manager or other staff when risk factors for social determinants of health are identified
  • You will perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs
  • You will identify and address barriers to discharge and recover, including financial, emotional, psychosocial, medical, and spiritual
  • You will provide education to patients and caregivers regarding community resources and postacute level of care criteria, benefits, and coverage
  • You will engage in community outreach and program development to support patient and caregiver needs
Job Requirements

Experience

  • Knowledgeable about internal, community and postacute care resources
  • Basic social worker skills including, but not limited to, empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention
  • Acute care social work experience preferred
Education

  • Master of Social Work (MSW)
Licenses and Certifications

  • State licensure through the Texas State Board of Social Worker Examiners
  • Certification in Social Work or Case Management preferred
