    Direct Home Care Worker

    Join our team at a growing Home Health Care Organization nestled in the heart of Michigan This thriving patient-focused facility is seeking a dedicated Direct Home Care Worker to enhance our patient care team.


    Responsibilities:


    • Under supervision, provides supportive services to participants in various settings.
    • Follows individual Care Plans for each participant.
    • Assists in maintaining physical and emotional comfort and promoting independence.
    • Performs a range of tasks including personal care, meal assistance, medication reminders, and transportation support.
    • Ensures accurate and timely documentation of care provided.
    • Responds to emergencies and reports deviations promptly.

    Qualifications:

    • High School Diploma or G.E.D. required, with a minimum of 2 years relevant experience or completion of a formal training program.
    • Participates in annual inservice training and screenings.
    • Demonstrates interpersonal skills and respect for participants and colleagues.
    • Flexible scheduling ability and sensitivity to diverse backgrounds.
    • Must meet standardized competencies and maintain medical clearance for direct participant contact.

    Working Conditions:

    • Requires driving within designated area with reliable transportation and valid driver's license.
    • Variable working conditions with potential exposure to communicable diseases and chemicals.
