    We are seeking a dynamic and experienced Consulting Case Manager to join our team.

    This role is ideal for someone with a strong background in healthcare, possessing deep medical knowledge and a firm understanding of treatment plans.

    Our ideal candidate will be a master connector, bridging the gap between our clients and the healthcare services they need.

    This role requires a deep sense of empathy, the ability to conduct comprehensive screenings and assessments, and the skills to implement effective strategies for client care.


    Responsibilities:
    As a Consulting Case Manager, your primary responsibilities will include:


    • Conducting comprehensive screenings and assessments to identify the needs of each client, taking into account their medical history, current health status, and future health goals.
    • Developing and implementing personalized treatment plans that address the specific needs of each client, ensuring they receive the highest quality of care.
    • Acting as a connector between clients, healthcare providers, and other relevant parties, facilitating communication and coordination to ensure seamless service delivery.
    • Continuously monitoring and evaluating the effectiveness of treatment plans, making necessary adjustments to ensure optimal outcomes for clients.
    • Providing clients with information and resources to help them navigate the healthcare system, advocating for their rights and needs when necessary.
    • Leveraging your medical knowledge to educate clients about their health conditions and treatment options, empowering them to make informed decisions about their care.
    • Demonstrating empathy and understanding in all interactions with clients, providing emotional support and reassurance during challenging times.

    Qualifications:
    To be considered for the Consulting Case Manager role, candidates must possess the following qualifications:


    • A minimum of 5 years of experience in a healthcare setting, preferably in case management or a related role.
    • Proven experience developing and implementing treatment plans, with a track record of achieving positive outcomes for clients.
    • Strong medical knowledge, with the ability to understand and explain complex health conditions and treatment options.
    • Excellent screening and assessment skills, with the ability to identify the needs of clients and develop personalized care plans.
    • Exceptional communication skills, with the ability to act as a connector between clients, healthcare providers, and other parties.
    • A deep sense of empathy, with the ability to provide emotional support and reassurance to clients during challenging times.
    • Experience in the healthcare sector, with a strong understanding of the healthcare system and the ability to navigate it effectively.
    • Degree in Nursing, Social Work, or related field is preferred.
