    PHYSICAL THERAPISTS for Home Health - FULL-TIME,PART-TIME,OR PER DIEM – San Jose Area. For Topaz Healthcare Services - Cupertino, CA

    Full-time roles are offered with a Competitive benefits package (including Health, Vision, Vacation, 401K).

    • Per Diem SOC/Eval/Visit Rate Range: $110-$150
    • Full-time role salary range: $120K-$160K

    H1-B Visa transfer available for suitable candidates

    Topaz Healthcare is based in Cupertino, CA. The company provides Physical, Occupational, Speech Therapists, Social workers, and Dieticians as service providers to large Healthcare Service agencies, Hospitals, and Senior Care facilities. Topaz HealthCare is seeking Healthcare professionals, Full-time, Part-time, Per diem Occupational Therapists in the South Bay, East Bay area, Alameda County, and Contra Costa County area in Northern California. We currently have openings in San Jose, Campbell, Mountain View, Hayward, Fremont, San Ramon, Gilroy, and Morgan Hill.

    Qualifications:

    • Perform Diagnostic, Preventive, and Rehabilitative procedures as required for the patient's care.
    • Ability to Create a Plan of Care by evaluation and assessment of patient's levels of functionality
    • Maintain continuity of and/or multidisciplinary services whenever needed to ensure good coordination of care.
    • Timely Documentation as per requirements for state, federal, and organizational guidelines.
    • Dependable, Good communication and Organizational Skills.
    • Highly motivated and passionate about working with the senior population with the ability to blend healthcare and hospitality services.
    • Home Health experience preferred but not necessary: One year


    Requirements:

    • Active Physical Therapy License in the state of CA: 1 or more years of experience
    • Dependable Transportation, Proof of a valid Driver's License, and Auto Insurance.
    • Current CPR certification
    • TB testing documentation.
    • Ability to perform the physical aspects of the role.
