    Job Description
    Physical therapy goals are to restore function to prevent or minimize disability
    following disease or injury and to help the client reach his/her maximum
    performance/function within the limits of his/her capabilities. The Physical
    Therapist evaluates and treats patients using medically prescribed physical
    therapy programs. The Physical Therapist may also supervise Physical Therapy
    Assistants per agency and regulatory standards.

    ORGANIZATIONAL RELATIONSHIPS:

    • For patient care issues, receives direction supervision from assigned
    Manager of Clinical Services (MCS).

    • For clinical rehabilitation issues, the same MCS will provide direction if they
    are licensed therapist. If not, The MCS who is a licensed therapist will
    provide clinical direction.

    • Receives annual evaluations from the Manager of Clinical Services (MCS).
    • Supervises and coordinates medically prescribed physical therapy programs
    for assigned Physical Therapy Assistant (s). (PTA)

    AREAS OF RESPONSIBILITY:

    • Makes initial admission/evaluation visits. Notifies assigned Clinical Team
    Manager of OASIS scores within established timelines. Notifies
    appropriate staff of admission/evaluation and provides information on
    follow up plan.
    o Performs an initial, comprehensive assessment which includes but
    is not limited to the patient's eligibility for home care services in
    accordance with third party payer regulations, an accurate
    reflection of the patient's current health status, review of all
    medications the patient is currently using, an environmental
    assessment which includes physical, social and mental status,
    identified needs and potential to reach treatment and discharge
    goals.
    o Documents observations, clinical findings, problems, skilled
    interventions/treatments , goals and discharge plans

    • In consultation with the assigned Manager of Clinical Services, initiates
    and regularly re-evaluates and revises plan of care. Instructs assigned
    PTA on plan of care revisions.


    ESSENTIAL PHYSICAL AND MENTAL REQUIREMENTS

    Physical:
    Must be able to drive to patients' homes, maneuver around
    objects, climb stairs or work in relatively small spaces as part
    of home visit use assessment skills, support, and move and
    lift patients of varying weights, stretch and bend as
    necessary, write treatment plans, progress notes and
    activities records.

    Sensory:
    Must be able to hear, read, write and speak clearly to
    effectively communicate in person and via telephone with
    patients, caretakers, staff and physicians etc.

    Mental:
    Must be able to understand supervisor's directions and make
    decisions related to patient care and have clinical knowledge
    and critical thinking ability to effectively plan and provide
    patient care and education consistent with standards and
    regulations.
