    Summary:

As aPersonal Outcomes Champion, you are entrusted with the responsibility ofensuring that each individual you support is afforded the same rights and respectedin the same manner as yourself. It is the expectation that this position willrepresent, advocate and pursue the dreams of the individual, making their voicethe loudest heard.

    Essential Functions:

    Meaningful Life:

    -Learnand practice the Principles of Everyday Lives when offering support toour individuals.

    -Embracethe Charting the Life Course assessment and create a life-map to helpsteer decision making and set individual goals.

    -Ensurethat the individual is always considered in decision making and that choice isexpected, not optional.

    -Explaineach individuals' Rights and the responsibility and risk those Rights carryuntil comprehension is achieved.

    -Createa relationship/connection with the individual so that you can comfortablydiscuss each person as an individual, not a case.

    -Determinewho the significant stakeholders are in the individuals life and establish ahealthy rapport with them.

    ProgramKnowledge, Skills and Responsibilities:

    -Oversee the coordinationof services for the individual and promote continuity and consistency in theirservice delivery

    -Complywith State mandated regulatory Program Specialist (known as PersonalOutcomes Champions at KenCrest) requirements for all programs that touch thelives of the individuals that are presented on your caseload.

    -Practiceand encourage professional and healthy communication with all persons who havean influence on the individuals on your caseload.

    -Ensurethat the individual's medical, dental, social, financial, behavioral, spiritualand personal needs, wants and desires are being accounted for and addressed.

    -Emergentsituations, upon discovery or notification, will be prioritized. Necessary and appropriate actions should alsobe considered a priority and communicated as such.

    Additional JobResponsibilities:

    -Activelyimplement the Standards of Practices established for the department

    -Participatein both required and optional professional trainings

    -HIPAAcompliance will be maintained at all times, including at the mobile andtouchdown workspaces

    -Maintainfamiliar and working knowledge of both regulatory and KenCrest expectations

    -Seekdirection and/or support from colleagues or Supervisors as necessary

    -Attenddepartment meetings as scheduled (in person or remotely)

    Minimum Qualifications:

    -ABachelor's degree in Human Services or related field from an accredited collegeor university and 2 years work experience working directly with individualswith intellectual disabilities preferred

    -ClearCriminal History and child abuse check in accordance with State regulatoryrequirements

    -ValidDriver's license

    -Reliable Transportation - this position requires that you drive yourself fromlocation to location daily.

    -Technologicalaptitude will be required, including the use of a laptop with programs such as MicrosoftOutlook, Word, Evolv) and a cell phone

    Physical Demands:
    • The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of the job. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions.
    • Regularly required to sit, stand and walk for periods of time, occasionally climb and descend steps, and frequently talk and hear. The employee is frequently required to use hands and fingers to handle office materials and or equipment. The employee is frequently required to drive from location to location for periods of up to 1 hour, occasionally required to drive for periods of up to 3 hours or more.
    • Be capable of visual and auditory scanning of the work environment so as to monitor the facility to ensure safe and effective office operations.
    * "Medium" physical work refers to the abilityto exert or lift up to 30 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10pounds of force constantly to maneuver objects.

    Working Conditions:

    This position required the use of yourpersonal vehicle to transport you from location to location. It is the expectation that you work remotely,using designated hoteling areas at KenCrest facility-based programs/offices/homes.

    We are requiring all new team members to be fully vaccinated with a COVID-19 vaccine*, except where a reasonable medical or religious accommodation can be granted. Should you be offered and accept aposition with KenCrest, you will be required to provide your proof of vaccination in the form of your CDC vaccination card as part of the pre-employment onboarding process.
