Job Description Summary
Responsible for the operation of the diabetes self-management education program, including supervisor of staff and program planning, development, implementation and evaluation

(Salary Range:

$35.79-$69.09)


ESSENTIAL FUNCTIONS

  • Provide overall coordination for the program including supervision of all instructors and clerical staff working for the diabetes self-management education program
  • Serve as a liaison among the program staff, advisory committee, and facility administration.
  • Provide and assist the orientation and continuing education for the program personnel.
  • Coordinates the planning and review of the program each year.
  • Participate in the preparation of program budget.
  • Design and carry out evaluation of program effectiveness.
  • Serve as the chair and attend all meetings of the advisory committee
  • Provide assessments, teaching and follow-up instruction to program participants.
  • Maintain licensure and continuing educational credits per ADA guidelines.
  • Other duties as assigned.

CDE functions:
a. Maintains all records in a current and accurate fashion.

b. Adheres to all requirements by various outside agencies and regulating bodies

c. Documents nursing interventions and ensures that the records are complete and appropriately signed.

d. Maintains logs needed for reporting purposes and statistical records in order to track trends and analyze pertinent data.

e. Documents telephone conversations with patients.

f. Uses correct procedure when faxing patient informatin.

g. Provides copies of documentation to referring healthcare provider(s).

h. Establishes priorities for patient care.

i. Safely handles biohazardous materials

j. Demonstrates proper body mechanics.


  • Understands emergency procedures and fire regulations.
a. Complies with hospital safety standards including universal precautions

b. Demonstrates knowledge of commonly used medications including action, dose and side effects

c. Uses resources to review new or unfamiliar medications.

d. Demonstrates knowlege of insulin administration for insulin-dependent patients, including various new insulin-delivery devices, systems.

e. Works to ensure a clean and safe environment at all times for patients and other staff members.


  • Professional Develoment
a.

Obtains regular continuing education in the areas of diabetes management, behavioral interventions, teaching, learning, and counseling skills per American Diabetes Association guidelines.

b. Participates in educational activities relating to cultural and behavioral change strategies.
