    Position: Patient Navigator I

    Department: PDD

    Reports To: Program Operations Supervisor

    POSITION SUMMARY:

    Navigators will deliver services to individuals that are being incarcerated in Lancaster at the Sheriff Department in Lancaster as part of the Alternatives to Incarceration Program. Operating within the justice involved demographic, key responsibilities including but not limited to providing transportation, advocating for patients, and coordinating essential services. The overarching goal is to contribute to the promotion resiliency, and self-sufficiency among individuals involved in Alternatives to Incarceration Program, fostering a supportive environment that facilitates positive outcomes and personal empowerment.

    Salary Range

    $ $27 / Hour DOE

    Shifts will include overnights and weekends as applicable

    Requirements:

    • High School diploma, or equivalent, is required.
    • Bilingual (English/Spanish) a plus
    • CA Driver's License.

    SUMMARY OF RESPONSIBILITIES:

    Position is responsible for completing preliminary screening to help determine level of care and services to support recovery efforts, as well to help individual access needed health and social support services. Position conducts on-site and/or field- based outreach, engagement, re-engagement and navigation services and scheduling.

    SPECIFIC DUTIES:
    • Navigation
    1. Conduct initial navigation screening on the phone or in person with the objective to help determining the need and services of each individual and respond to referral sources from organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others.
    2. Provide enhanced patient navigation services to identify and link eligible participants to project services.
    3. Participate in outreach efforts at community sites and in-reach at TTC service site.
    4. Maintain accurate and timely information in the Access Log to document all contact and navigation encounters with individuals or referral sources.
    • Scheduling
    1. Receive warm transfers from navigators and schedule patients for intake via the Electronic Health Record (e.g., Avatar Scheduling Calendar for behavioral health)
    2. Maintain accurate and timely notes to document all attempts at scheduling and encounters with the individual and referral sources in the Access Log and follow up with pending Scheduling Assignments in the Access Log to ensure all scheduled appointments and linkages are completed.
    • General
    1. Provide excellent customer service, and timely response times during scheduled call center shift.
    2. Provide weekly reports of new individuals enrolled in navigation services to the immediate supervisor.
    3. Provide advocacy as needed to help address issues and barriers between patient and service providers.
    4. Maintain a database of patients served and outcome data for reporting purposes.
    5. Attend agency and community meetings as directed by supervisor.
    6. Execute additional tasks, projects and responsibilities as assigned.

    MINIMUM QUALIFICATIONS:

    KNOWLEDGE AND SKILLS:

    • Passion and desire to work justice involved demographic.
    • Demonstrate a non-judgmental attitude and positive regard for justice involved demographic???????.???????
    • Experience providing field-based services.
    • Strong people skills and ability to work with people of various ages, racial, ethnic, cultural, educational and socio-economic backgrounds.
    • Organized, detail oriented and creative problem-solving.
    • Proficient in computer skills and use of Microsoft products such as Outlook, Word and Excel.
    • Must have valid CA driver's license.
