Job Summary

Supports Case Managers with administrative tasks and client transportation, aiding in the coordination of a comprehensive care delivery system for all clients.

Key Responsibilities

  • Offers pertinent feedback about clients to Case Managers to assist in monitoring ISP outcomes, guiding reassessment or revision of ISP as needed.
  • Aids Case Managers in delivering supportive interventions to clients, including direct teaching for adult-living skill development and maintenance. Contributes to maintaining a clean workspace for staff and clients.
  • Transports clients to medical and essential appointments indicated by the Case Manager.
  • Assists in all necessary duties related to program and administrative data upkeep, record-keeping, and data collection activities as outlined by Rushford.
  • Provides immediate crisis intervention when required, referring clients to the Crisis Program, Case Manager, or Clinician as appropriate.
  • Performs any other necessary tasks as required.

Qualifications

  • Associate's degree in a mental health or human service discipline or relevant life experience.
  • Must hold a current, valid Connecticut driver's license. Eligibility for a Public Service License is essential.
  • Previous professional experience with individuals dealing with severe and persistent mental illness.
