    Home Care Representative Associate

    to join a growing and dynamic team

    Job Description:
    Assist the Home Care Representatives to facilitate referrals for Medicare Certified home care services to ensure appropriate continuity of patient careCollaborate and assist internal and external customers to provide a comprehensive referral for cliniciansPerforms data entry functions as neededCommunicate both in person and over the phone with customers patients and families and referral sourcesOperate office equipment including computer, telephone, copier, and facsimile machine/serverAssist with the electronic referral intake processMaintain current knowledge regarding insurance coverage guidelines for home health services; obtains prior authorization or start the process as required to initiate careAttend staff meetings, professional service meetings and participates in staff development activities and workshops as required

    Position Requirements:

    Must have knowledge of medical terminologyAbility to be accurate and meticulous when completing assignmentsAbility to problem solve quickly and efficiently, identifying and solving problems with easeInterpret, adapt and apply guidelines and proceduresObtain clear communicateMust be fully Covid-19 vaccinated; those with eligible exemptions will be tested regularlyMust have current TB clearance.

    Valid drivers license, vehicle, vehicle insurance as required by state law.
