    L.I.F.E. , Inc. Community Living -Program Coordinator Job Summary: Under the general guidance of the Director of Community Living and the direct supervision of the Assistant Director of Community Living this individual will be responsible for the day to day supervision and management of assigned community living programs.

    This position provides critical services in ensuring person-centered services are delivered in a manner that is consistent with individuals preferences and desires; delivered according to established practices within the Developmentally Disabled (DD) community. The role of the Program Coordinator is to serve as a hands-on team member while providing administrative and programmatic support within the program. This include coordinating and scheduling of staff and overseeing transportation efforts when necessary.

    This position will provide on-call duties as identified by the agency guidelines. In summary, this person provides administrative, programmatic and management supports to an assigned group of homes or apartments. This position supervises House Managers, ensures programs are meeting waiver and non-waiver guidelines. This position applies best practices to ensure programs are meeting standards as identified in the final rule.

    Essential Functions:

    Maintains responsibility for the assigned participants through weekly inspections assuring that the safety and health of the individuals homes meets the standards of L.I.F.E., the State and Local Regulatory agencies.


    • Establishes and supervises the scheduling of staff to ensure that all necessary safety and program issues are implemented.


    • Under the direction of the Community Living Assistant Director hires, trains, supervises, evaluates and terminates, when necessary, staff under your supervision.


    • Coordinates and participates in individual plan meetings (IP) and other inter-disciplinary meetings with agencies participating in the individuals overall program. Guarantees notification to individuals extended program participants such as biological family, surrogate family, guardians, legal counsel educational and or vocation program and CLP staff.


    • Participates in the admissions discharge and review (ARD) process within the restrictions established by COMAR regulation insuring that admission, annual and discharge timeframes are met, that resulting documentation reflects COMAR standards and that the plan established as a result of the Individual Plan (IP) is implemented in a timely period.


    • Assures the accuracy of assigned staff's payroll and assists them in following established fiscal guidelines when and if necessary. Processes petty cash and banking/budget information, ensures that petty cash receipts and purchases are accurate.


    • Functions as the Community Living Program (CLP) agent in the region where assigned and represents the agency on various committees and within the community at large


    • When necessary acts as the individuals advocate and assures that all rights, interests and preferences are being addressed.


    • Responsible for promoting a culture of collaboration and person-centered practices that ensure individuals served are the drivers of their life decisions


    • Responsible for assuring that medication ordered by physicians are received in a timely and accurate fashion. Maintains oversight of general health records and guarantees the individual's right to competent health services.


    • Responsible for monitoring and reviewing of Medication Administration Records (MAR)'s consistent with agency standards and practices


    • Assists with other administrative responsibilities and other duties as assigned.

    Qualifications:


    • Minimum Associates degree


    • 2 years of supervisory and administrative experience preferably in residential services in the field of Developmental Disabilities.


    • Access to a car and have a valid Maryland Driver's License.


    • Demonstrated ability to work independently, as a team member, and to promote quality programs.


    • Basic proficiency in office suite, to include word, excel etc

    Other Requirements:


    • Valid Maryland Driver's License
    • Access to reliable transportation


    • This position requires travel within the program


    • Successfully pass background check/ and drug testing

    Physical Requirements


    • Must be able to pass MANDT physical and mental requirements


    • Must be able to read and write using the English language


    • Must possess the visual acuity to develop written correspondence and determine the accuracy and thoroughness of work assigned


    • Must be able to physically perform the basic life operational functions of reaching, lifting, talking, hearing, sitting for repeatedly and for long periods of time
