- Documentation of group goals, objectives, and progress.
- Refers and educates patients/families regarding community resources and provides follow-up.

- Supports performance improvement initiatives based on patient satisfaction and quality outcomes.
- Demonstrates responsibility for own professional growth and development.
- Responsible for tracking data for grant reports and following all grant requirements.
- Organizes and leads the youth advisory council, with the goal of obtaining input from the students and supporting school wide initiatives.
- Promote and spread awareness of the school-based sites.
- Health Education presentations to school staff, students, and families.
- Demonstrates knowledge and commitment to the mission, philosophy, and objectives of the Corewell CHAC/Teen Ctrs/SWP/E3 sites and the Community Benefit/Health Division.

- Bachelor of Social Work

GENERAL SUMMARY:
The School Based BSW collaborates with members of an interdisciplinary team to assist patients and families with complex psychosocial needs.
The School Based BSW will work within 3 schools to provide evidence-based curriculum on topics that may include but are not limited to, grief, healthy relationships, anger management, suicide prevention, depression, and anxiety.
Other tasks include case management, referrals to community-based programs, and follow-up.ESSENTIAL DUTIES:
