School Based Clinic Social Worker BSW - Flat Rock, United States - Corewell Health
Description
GENERAL SUMMARY:
The School Based BSW collaborates with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. Corewell Healths Expanding, Enhancing, Emotional Health Programs are located on school grounds. Staff members work closely with school administration to provide interventions to students during the school day. The School Based BSW will work within 3 schools to provide evidence-based curriculum on topics that may include but are not limited to, grief, healthy relationships, anger management, suicide prevention, depression, and anxiety. Other tasks include case management, referrals to community-based programs, and follow-up.
ESSENTIAL DUTIES:
1. Promotion, development, recruiting, evaluation, and facilitation of evidence-based curriculum.
2. Documentation of group goals, objectives, and progress.
3. Refers and educates patients/families regarding community resources and provides follow-up.
4. Engages in professional consultation and supervision.
5. Supports performance improvement initiatives based on patient satisfaction and quality outcomes.
6. Demonstrates responsibility for own professional growth and development.
7. Responsible for tracking data for grant reports and following all grant requirements.
8. Organizes and leads the youth advisory council, with the goal of obtaining input from the students and supporting school wide initiatives.
9. Promote and spread awareness of the school-based sites.
10. Health Education presentations to school staff, students, and families.
STANDARD REQUIREMENTS
1. Demonstrates knowledge and commitment to the mission, philosophy, and objectives of the Corewell CHAC/Teen Ctrs/SWP/E3 sites and the Community Benefit/Health Division.
2. In encounters with staff, patients, and their families, demonstrates Compassion, Collaboration, Clarity, Curiosity, and Courage.
3. Demonstrates respect and maintains clients dignity and confidentiality.
STANDARD QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 1. Education / Training:
Bachelor of Social Work
2. Work Experience:
A minimum of 2 years experience working with youth, preferably with experience as a group facilitator.
3. Certification, Licensure, Registration:
LLBSW, LBSW
4. Other Qualifications: Demonstrate knowledge of growth and development and dynamics of behavior for age related groups. Must demonstrate excellent oral and written communication skills. Must be proficient in Microsoft Word and ability to adapt to technical demands of electronic communication. Ability to work independently but contribute positively to team performance. Ability to manage, prioritize multiple situations and tasks while remaining calm. Must demonstrate willingness to be flexible and cope with multiple job stressors.
