Description
CORD's Transition to Adulthood Program (TAP) assists high school students ages 14-21 in the development of their independent living goals.
This includes developing Independent Living Plans (ILP's) as well as providing peer support, skills training, and individual advocacy to high school students with significant disabilities of any type who are receiving special education services.
The program requires individual, group, and community activities with emphasis on one-on-one consumer work.ESSENTIAL FUNCTIONS:
- Review IEP's and attend school team meetings to advocate for students' rights in education and transition services;
- Conduct ongoing outreach activities to schools and the community at large to drive intakes/enrollment in the program;
- Provide informational workshops and forums for consumers, family members, school personnel, and other service providers;
- Work with consumers individually on the development of IEP's and goals for transitioning out of special education;
- Attend regular CORD staff, program, and other team meetings; and
- Perform community activities through CORD's mobile food pantry at least one day per week as directed.
Pay:
$22.00 per hour
