Outpatient Services Quality Assurance Associate - $17.41/Hr

Mon - Fri, 8:00 am - 4:00 pm


Duties and Responsibilities:


  • Supports Nurse Practitioner by:
  • Running reports as requested
  • Scheduling appointments
  • Conducting all reminder calls for Nurse practitioner appointments
  • Faxing as needed for medication refills
  • Screening fax folder daily and following up with APRN/Clinician
  • Scans appropriate intake documents into Electronic Healthcare Record and requests signatures on documents as designated
  • Screens interested service applicants for intake as designated
  • Provides information on local community services, as needed, when inquiries are not necessarily appropriate for RH
  • Answers RHOS initial inquiry calls and completes short screening tools for clinical review
  • Schedules outpatient appointments and coordinates schedules for clinical programs as needed
  • Provides reminder calls to people scheduled for outpatient services
  • Conducts discharge calls, discharge surveys and creates discharge letters
  • Provides additional data entry into electronic medical system as needed
  • Reviews program enrollment for accuracy and communicates issues to QA personnel for correction
  • Conducts followup calls for missing insurance information for outpatient services, as needed
  • Establishes and maintains confidentiality according to HIPAA regulations
  • Works at different locations per Agency need
  • Engages in continuous concerted efforts to access, obtain, respond to pertinent Agency information/communications, and disseminate as appropriate
  • Provides conditions that maximize safety for coworkers and program participants
  • Other responsibilities of Reliance Health staff, such as participation in paid Agency events, adhering to Agency Policies, training requirements etc.
  • Performs duties while demonstrating Agency Core Competencies and Values

Education and Experience:


  • High school diploma or equivalent
  • 2 years previous work experience in front desk experience and quality management in medical field preferred
  • Experience in medical terminology preferred
  • Experience working for nonprofit agency preferred
  • Valid CT Driver's License, vehicle registration and insurance
