+Responsibilities
- Provide support to REACH clients who are hospitalized at state or local hospitals within the DBHDS.
- Serve as the primary contact for discharge planning and facilitates admission to the REACH Crisis Therapeutic Home or REACH Mobile Crisis Supports, if appropriate.
- Competitive salary: $60,,000 annually
<b>Qualifications</b>
- Minimum of a bachelor's degree in human services or related field
Must obtain, or be eligible to obtain, QMHP-T registration with the Virginia Board of Counseling within 30 days of start.
Licensed or license-eligible as an LPC, LCSW, or LMFT preferred
Crisis intervention adaptability problem-solving decision making
Knowledge of Virginia Hospital & Community Based services required
Maintain a valid driver's license good driving record current auto insurance.
P roficiency with EHR systems Microsoft Suite technically savvy
