Position Description

Overview

The Care Coordinator reports to the VP of Quality and Risk Adjustment with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements.

Responsibilities:


The Care Coordinator will have the following responsibilities:


  • Patient Care Coordination: interact with members on a day-to-day basis to assess their health care needs and assist with the logistics of ensuring they have the needed care.

Patient Education: Educate members and their families about their services and benefits available to them through our program, our primary care network, and their health plan. Ensure members understand and follow recommended care plans.

Appointment Scheduling: Arrange and schedule medical appointments, tests, and consultations for members. Coordinate transportation and accessibility requirements for members as needed.

Resource Referral: Connect members with community resources, support services, and financial assistance programs. Assist in obtaining necessary medical equipment and supplies.

Documentation: Maintain accurate and up-to-date patient records, including treatment plans and progress notes. Ensure compliance with privacy and confidentiality regulations.

Advocacy and Support: Act as an advocate for members, addressing their concerns and assisting in navigating the healthcare system. Provide emotional support and guidance to members and their families.

Communication: Foster effective communication between members, families, healthcare providers, and other relevant stakeholders. Report any concerns or issues to the healthcare team promptly.

Quality Assurance:Participate in quality improvement initiatives to enhance the care coordination process. Stay current with healthcare best practices and industry developments.


Qualifications

The Care Coordinator should have the following qualifications:

Education:


  • High school diploma required.
  • Certification as a Medical Administrative Assistant, Community Health Worker, Patient Care Technician, or Bachelor's degree in Healthcare, Social Work or equivalent is preferred.

Experience:Prior experience in a healthcare setting, managed care setting, care coordination program, or related experience required.
