- __ Welcomes patients/clients by offering service, assistance, and determining the reason for their service to facilitate activities to meet their needs
- Reviews clinical information, troubleshoots complex patient referrals, and discusses with appropriate personnel to ensure referral is appropriate. Enters and obtains orders for referrals
- Completes the insurance verification process
- Assigns and coordinates teams for all admissions, accommodating patient and physician requests. Works collaboratively with healthcare facilities and physician offices in support of the admission process
- Accurately informs team of expected discharge date of patients, works collaboratively with all areas in support of the scheduling process, and acts as a patient resource in the scheduling of follow-up procedures/appointments
- Performs system maintenance and clerical duties, including merging patient information when two accounts exist, loading new doctor and facility information into system. Running and updating reports as needed and validating physician licenses

The Central Intake Licensed Practical Nurse, LPN registers and schedules all Home Care patients/clients. This role coordinates the intake process with physician offices, emergency departments, and other healthcare facilities and ancillary areas. They assist with insurance verification and related support duties.
In addition to central intake processing, the Central Intake LPN performs necessary clinically related functions to support the intake/admission process.
