Amelia Home Care, Inc is seeking a dedicated Care Coordinator to join our team. The Care Coordinator will play a crucial role in supporting the coordination of care for patients. This position offers the opportunity to work in a dynamic healthcare environment.


Responsibilities:

• Staff open cases
• Coordinate patient care activities to ensure appropriate followup and adherence to care plans
• Communicate with the HHA/PCA'S on daily basis and follow up with them for missing timesheets
• Communicate with healthcare providers, patients, and families to facilitate continuity of care
• Document all notes on HHA exchange
• Responsible fro completing incident/update reports
• Dashboard mainteance: Monitor HHA exchange to confirm daily missed visits and weekly prebilling updates
• Responsible for on call coverage to ensure HHA placement
• Assist in obtaining authorizations for medical procedures and services
• Provide medical administrative support as needed
• Collaborate with medical and administrative staff to optimize patient care

Qualifications:

• Must have home care coordinating experience**_

Pay:
$ $23.74 per hour


Expected hours:
No more than 40 per week
