Responsibilities

Perform in-home and ensure visit time fully meets the needs of the patient.
Coordinate total patient care including interdisciplinary communication with other health care providers to enhance continuity of care.
Render skilled nursing care, as assigned by the registered nurse, according to the physician's orders.
Recognize and interpret symptoms; report patient's condition and assist with or institute remedial measures for adverse developments.
Guide and instruct patient and their families in prescribed activities that are directed toward improving independence and functionality.
Ensure lab tests are obtained, stored, and transported according to acceptable laboratory practice and OSHA guidelines.
Review and update the medication profile; assess patient compliance, understanding of medication regime, contraindications, and side effects.

Qualifications

Must be an LPN or LVN in the state of employment.
Must have at least one year of experience as an LPN or LVN.
Medicare home health experience is preferred.
Must possess a valid state driver license, maintain automobile liability insurance as required by law, maintain dependable transportation, and must be able to safely drive in all types of weather.
Must possess CPR certification for the healthcare provider.
