It's our mission to serve the most vulnerable seniors with better quality and compassion in a value-based model.The Home Health LVN plays a vital role in achieving our mission by performing skilled duties at home under CMS approval, bringing convenient and necessary care to our participants' homes.
Under the supervision of WelbeHealth RNs, NP or MDs, the Home Health LVN is responsible for carrying out tasks in the participants' residences as assigned such as home safety assessments, data gathering and supervisory visits.
The Home Health LVN will perform skilled duties only after site licensure or in the event of pandemic/stay at home order with CMS approval to provide necessary care in the home.
Essential Job Responsibilities:
- Provide and document inperson supervision and instruction to home care assistants and home health aides assigned to participants' homes at least as frequently as specified in the Home Health Agency regulations and more often if necessary
- Instruct the participant, family and caregivers regarding the disease process, selfcare techniques, and prevention strategies, as assigned
- Conduct and Perform home safety assessments as assigned
- Deliver and document nursing interventions as assigned, accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments
- Support to the clinic staff and home health staff work to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
- Must be willing and could work a varied schedule that may include evening nights, weekends and overtime
Job Requirements:
- Graduate of an Accredited School of Nursing Required
- Unencumbered LVN License in state practicing
- Reliable means of transportation
- Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations
