RN Case Manager I Overview:

The RN Case Manager plays a vital role in coordinating care for patients, ensuring optimal health outcomes, and efficient resource utilization. Responsibilities include utilization management, transition planning, care coordination, compliance with regulatory standards, and patient education. If you are a dedicated RN looking to make a meaningful impact, consider joining our team.

Requirements:

Education: Graduate of an accredited nursing school
Licensure/Certification/Registration: Active RN license
Preferred: Accredited Case Manager (ACM) certification
Work Experience: 3 years of department experience or 4 years of acute hospital case management
