JOB DESCRIPTION:


Summary of the Position:


The Case Manager Supervisor will be responsible for the specific caseload of tenants at the New Chardon residence under the Behavioral Health Division of Pine Street Inn, a program that provides permanent housing to formerly homeless women using a Housing First/Stabilization model.

The program serves women from diverse backgrounds, circumstances, and age groups.

The mission is to provide trauma-informed, gender sensitive and gender specific support services that meet the special needs so they can heal and have time to heal, prepare to reintegrate into the community, and break the cycle of homelessness.

Tenants will be assessed for their actual level of functioning in various spheres upon entering the program and supported in individual plans to improve and enhance this level.

The Case Manager Supervisor is responsible implementation of effective best practices (Motivational Interviewing, Comprehensive Case Management, Harm Reduction, and Trauma - Informed Care) and ensuring the provision of high quality case management supports on a 24/7 basis.

The Case Manager Supervisor will directly supervise and provide support to a Case Manager and an Activities Coordinator.

The incumbent will be a great team player who is also self-motivated, with solid communications skills, flexibility and creative problem solving.

Qualifications:


Education/Training:


Required:

  • Bachelor's Degree in Social Work or other related field such as Psychology, counseling, psychosocial rehabilitation

Preferred:


  • A valid Massachusetts Driver's License in good standing
  • Master's Degree in Social Work or other related field such as counseling, Psychology, psychosocial rehabilitation
  • Advanced training in psychosocial rehabilitation and substance abuse


  • Bilingual

  • Spanish/English

Knowledge/Experience:


Required:

  • Minimum of at least (2) years' experience working as a Lead Case Manager and/or coaching and mentoring and/or providing supervision to staff in the human services and related fields
  • Minimum of three (3) years prior work experience working with homeless individuals suffering from complex Trauma, SUD diagnosed, disabilities in an housing settings
  • Strong clinical background and familiarity with providing Trauma Informed Care and Harm Reduction Treatments to dual diagnosis populations
  • Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment
  • Experience conducting psychosocial assessments
  • Experience with case management and implementing treatment plans
  • Knowledge of treatments for chronic mental illness, including medications
  • Previous experience working with dually diagnosed individuals
  • Experience assisting people with ADL skills
  • Experience working with vendors and services providers within the public mental health, housing and SUD systems
  • Previous experience in providing support for individuals experiencing homelessness and mental health issues into housing

Preferred:


  • Previous experience providing Clinical supervision to staff in the human services or related fields
  • Previous experience working with a Housing First/Stabilization model
  • Familiarity with the Metropolitan Boston public and private mental health system
  • Experience working with individuals experiencing Long term mental health issues

Essential Functions Include But Are Not Limited To:

  • Carry a caseload of 10-20 tenants and meet regularly with each individual tenant to address barriers and provide support services to address needs.
  • Work with tenants to create clientcentered, strengths
- based individual treatment plans and periodically evaluated and edited as necessary for best practice and in accordance with PSI program standards

  • Perform ongoing clinical assessments, updating documentation and maintaining and updating tenant charts.
  • Have daily discussion with program staff to promote program integrity and to proactively identify and develop plan/intervention to address any issues
