    Job Description

    As a proactive, encouraging, and creative professional with a dedicated attitude, the Community Care Coach is responsible for providing trauma-informed community development and coaching-based case management to Bedford Dwellings residents through a Trauma Informed Community Development (TICD) framework. TICD establishes and promotes resilient healing and healthy communities so that individuals can be healthy enough to sustain opportunity and realize their potential.

    The Community Care Coach with Macedonia Family and Community Enrichment Center will collaborate with families to build trusting relationships, assess strengths and opportunities, address pressing needs, develop action plans for long-term objectives, and offer information and referrals to community-based partners. Playing a crucial role in fostering collaborative partnerships on behalf of residents, the Community Care Coach contributes to maintaining and strengthening these relationships. As part of a multidisciplinary team of Health, Education, and Workforce Liaisons based at Bedford Dwellings, the Community Care Coach will provide essential support to residents as the community undergoes redevelopment and revitalization.

    Responsibilities:

    • Build trusting relationships with residents through consistent outreach, prompt responses, and addressing immediate needs.
    • Conduct detailed coaching-based and person-centered assessments at the program's onset.
    • Perform comprehensive reassessments at least annually and update information quarterly.
    • Identify goals and create action plans related to education, health, and employment.
    • Provide resources, referrals to services, and ensure successful connections.
    • Offer ongoing service coordination, accompany residents when needed, and assist in accessing complex systems.
    • Adapt support levels based on residents' needs and interests.
    • Document efforts and outcomes using the Client Path data system.
    • Maintain organized records for each family.
    • Participate in Team meetings, Case Conferencing, Community meetings, and Resident meetings.

    Qualifications:

    • Passion for serving and empowering diverse, marginalized groups.
    • Bachelor's degree in Psychology, Counseling, Social Work, Public Administration, or related field is preferred.
    • 1-2 years of full-time professional experience in case management, community organizing, or community health.
    • Experience collaborating with diverse agencies and community groups.
    • Strong conflict management skills.
    • Ability to work collaboratively and convey empathy effectively.
    • Reliable transportation.
    • Independent work ability and time management skills.
    • Proficiency in Google Suite and Microsoft Office Suite.
    • Act 33, Act 34, and criminal background clearances required.

    SALARY: $40,000 to $50,000 range, commensurate with experience. Hybrid position.
