COMPANY PROFILE:
This organization is a Medicare Advantage Institutional Special Needs Health Plan (ISNP) for long-term nursing home residents. The plan covers all medically necessary and preventive services covered under Medicare Parts A, B, and D. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.
POSITION DESCRIPTION:
The APP is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.
The APP conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients' goals and needs of care. The APP is supported by a Clinical Manager and Clinical Director, as well as a local Medical Director.
Specific responsibilities include, but are not limited to:
The clinical work is autonomous, with one-on-one patient visits and a high level of independence. But it's also a team-oriented position with meetings, conference calls, etc. with members of the care team.
There are no nights or weekends involved. There may be some flexible, minimal on-call duties, but this will be discussed collaboratively and on-call volume is low, if required at all.
