    Gracepoint is one of Tampa Bay's leading providers of mental health solutions. Our organization provides 360-degree care from intake to primary care to meet the needs of children, families, and adults within the Hillsborough County community. Our Behavioral Health Network Program (B-Net) services children ages 5-18 (children served until age 19 years) under mental or behavioral health diagnosis providing individual and therapeutic support.

    POSITION SUMMARY:

    The Care Manager provides advocacy, coordination, and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. Participates as a member of a multidisciplinary treatment team—and reports to the unit/department supervisor.

    QUALIFICATIONS:

    Bachelor's degree in a Human Services related discipline and at least 1 year experience in the mental health field. (Child welfare programs require 2 years of experience with at-risk populations and a child welfare certification.) Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.

    FUNCTIONAL RESPONSIBILITIES INCLUDE:

    • Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
    • Complete all clinical and administrative documentation based on established timelines as per program requirements.
    • Communicate effectively and maintain cooperative relationships with internal and external resources.
    • Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements.
    • Maintain efficient workflow management and organization as per program requirements.
    • Maintain proficient quality of clinical documentation as per program requirements.
    • Other duties as assigned or needed as per program requirements.

    PHYSICAL DEMANDS:

    Abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. You may be required to complete standing presentations.

    WORK ENVIRONMENT:

    High-paced, collaborative team performance with an emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate-controlled office environments with frequent exposure to clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.
