Qualification:

Required Certification/License:
Must hold a valid certification as a Home Health Aide (CHHA) in the state of New Jersey.

Duties and Responsibilities:

Assist with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.
Aid in mobility and transfer activities, as necessary.
Provide support with medication reminders and monitoring.
Prepare and serve nutritious meals according to dietary restrictions and preferences.
Perform light housekeeping tasks such as laundry, vacuuming, and maintaining tidy living spaces.
Offer companionship and emotional support to clients.
Communicate effectively with clients, their families, and healthcare professionals.
Accurately and promptly document care provided and any changes in the client's condition.

Reporting Structure:
The Certified Home Health Aide will report to the Nurse Supervisor and Director of Nursing.

They will engage in close collaboration with these professionals to ensure the delivery of the highest standard of care to our clients.

Additionally, they will receive ongoing guidance, training, and support from the nursing team to continually enhance their skills and knowledge in home care practices.
