- **Member Assessment**:
  • Conduct comprehensive health assessments for adult day health members, considering their physical, emotional, and social well
  • being.
  • Identify and evaluate members' health conditions, ensuring a holistic understanding of their needs.

Care Planning:


  • Develop individualized care plans based on the assessed needs and goals of members.
  • Collaborate with members, their families, and other healthcare professionals to create and implement effective care strategies.

Health Monitoring:


  • Monitor participants' health status regularly and provide ongoing evaluation of their response to care interventions.
  • Administer medications and treatments as prescribed, ensuring compliance with all relevant protocols.

Collaboration and Communication:


  • Work closely with other team members to coordinate and integrate care services.
  • Communicate effectively with participants and their families, providing education and support to promote selfmanagement of health conditions.

Documentation:


  • Maintain accurate and uptodate electronic health records, documenting assessments, care plans, interventions, and member progress.
  • Ensure compliance with regulatory standards and organizational policies.

Health Promotion:


  • Implement health promotion activities to enhance members' overall well
  • being.
  • Provide education on disease prevention, selfcare, and lifestyle modifications.

Qualifications:


  • Current Registered Nurse (RN) license in the state of practice.
  • Leadership and Administrative Skills
  • Experience working with adult populations, preferably in a day health or similar communitybased setting.
  • Strong assessment, communication, and collaboration skills.
  • Knowledge of geriatric and chronic disease management.
  • Dementia Experience
  • Ability to work independently and as part of a multidisciplinary team.
  • Must have a contagious interactive, creative, and charismatic personality
  • Computer Proficiency
  • CPR and First Aid
  • Fingerprint Clearance
  • Freedom from TB

Education:


  • Bachelor of Science in Nursing (BSN)

Work Schedule:


  • Monday-Friday
