    $31.24 per hour.

    The role of the Case Manager involves offering case management, classification, and transitional services to individuals in confinement/residence.

    • Contribute to the initial assessment process by gathering, arranging, and assessing data about individuals.
    • Deliver direct casework services for social engagement, health requirements, and community integration to individuals in confinement/residence.
    • Compile and collaborate on various reports and create case presentations for designated individuals in confinement/residence.
    • Organize suitable programs, resources, and services for social growth and in readiness for the transition or release of individuals into the community.
    • Act as the main point of contact in the absence of the Unit Manager, as needed.

    Requirements:

    • Must hold a Bachelor's degree from an accredited university or college.
    • Prior experience in delivering case management services is beneficial. Additional background in a criminal justice or case management field can be considered in place of the educational prerequisite on a one-to-one basis for up to two years.
    • Proficiency in clear verbal and written communication is essential.
    • Proficiency in Microsoft Office applications or similar software is necessary.
    • Possession of a valid driver's license is mandatory.
    • Minimum age requirement: Must be a minimum of 21 years old.
