    JOB DESCRIPTION

    JOB TITLE: Care Manager Supervisor


    HOURS: Full time, Non-Exempt


    The Care Manager Supervisor is an integral part of Coastal Horizons Center's team approach to integrated care for behavioral health and medical care.

    The Care Manager (CM) Supervisor's primary function is to provide support, guidance and supervision of Care Managers as they work in partnership with individuals with behavioral, medical, and social determinants of health (SDOH) needs to coordinate care and services needed to assist individuals in achieving optimal health, wellness, and life goals.

    The Care Manager Supervisor is responsible in providing direct oversight of Care Managers to ensure provision of high quality services including comprehensive care management, care coordination and health promotion, comprehensive transitional care, individual and family support, referral to community and social support services, and use of Health Information Technology to link services.


    Qualifications / Skills:

    Required:

    A master's-level fully Licensed Clinical Social Worker (LCSW), fully Licensed Clinical Mental Health Counselor (LCMHC), fully Licensed Psychological Associate (LPA), fully Licensed Marriage and Family Therapist (LMFT), or licensure as an RN; three years of experience providing care management, case management, or care coordination to the population being served.


    Preferred:
    Commissioner for Case Management Certification (CCM)

    Ability to act quickly, assess and act accordingly in crisis situations. Basic technology skills and understanding of health records. Knowledge of ethical and professional responsibilities and boundaries. Demonstrate professional work habits including dependability, time management, independence and responsibility.


    Amount of Travel and any other
    Special Conditions:    Case Managers will be required to travel within the community in order to fulfill job related responsibilities (community engagement, care team meetings, etc.) Estimated on average 5% of time (2 hours per week)


    Salary:
    Range is on dependent qualifications and experience. Compensation is from $60,000 USD to $65,000 USD per year.
