Position Description:


The Youth Services Case Manager works with youth and their families who participate in RIL programs and are in need of behavioral health and social services.

The YSCM is tasked with developing a supportive relationship with the youth and the family to assess youth and family strengths and needs and help them to locate, access and utilize resources.


Services to be provided include home visits and school visits, assisting with coordination of care, and assisting youth and families in accessing needed resources (e.g., health services, food banks, social/recreational, employment).

This individual will coordinate and monitor the efficacy of resources in relation to both prevention and treatment; work collaboratively with the Articulation Navigation Team, Group Facilitators and Mental Health Clinicians, and provide follow up to help maintain progress notes in individual and group therapy sessions.

Requirements:_


Bachelor's degree in social work, Psychology, Sociology or Criminal Justice, or combination of youth services and/ or case management experience and education.

Must be able to maintain a flexible schedule to allow for evening appointments group meetings and occasional organizational events.

Be passionate about the work of Realism is Loyalty. Possess a passion for personal growth and giving others a hand up. Be able to set goals, implement process to meet those goal and manage time appropriately. Be a contributing team member, giving and accepting input and critiques of other team members.

Experience_:

Experience with at-risk/ under-served youth and their families required. Prior Case Management or client services experience a plus.

Knowledge and Abilities_:


This is a case management position with relationship building and connection to needed resources being the core element of this position.

Must demonstrate an ability to work successfully with the underserved and sometimes hard to deal with populations targeted by Realism Is Loyalty.

A broad-based working knowledge of community relationship and engagement as well as knowledge of other community-based resources are required. Must have basic computer skills and experience using Microsoft Office.

RESPONSIBILITIES:


CASE MANAGEMENT

  • Develop process for and provide resource assistance with participants and their families, ensuring it is effective, efficient and provided when needed by working closely with Mental Health Clinicians, Articulation Navigators, Group Facilitators, etc. to identify needs.
  • Upon initial interview, document participant's immediate needs, resources and services in collaboration w/mental health team; complete immediate Action Plan (shortterm goals/stepsto achieve needed resources/services).
  • Implement ongoing meetings w/participants to identify, address and/or remove specific barriers to attainment of needed resources.
  • Make referrals to community agencies to assist with resource needs.
  • Provide bus passes to participants as needed.
  • Transport participants, when appropriate, to support basic needs attainment.
  • Develop, with each assigned youth, a strengthsbased service plan addressing the needs for treatment, educational, vocational, social, and living skill development.
  • Observe youth during groups and utilize the information gleaned to share information with program and mental health team.
  • Utilize observation data to discuss service plan progress with youth/ family.
  • Ensure assistance with resource connection is comprehensive, including behavior health/substance abuse treatment and education needed by youth and family.
  • Coordinate and monitor the efficacy of resources in relation to both prevention and treatment.
  • Work in strong cooperation with school personnel to identify and respond to students and their families in a systemsfocused way.
  • Responsible for overseeing the continuity of care of youth assigned, especially during transitions to different schools, service providers and/or changes which may affect continuity of care.
  • Be available and responsive to all emergencies and crisis situations of assigned youth. Mobilize all resources available to assist youth in crisis.
  • Contact and engage previous participants who have recently disengaged services; ensure warm handoff to alternate service providers.

  • Attend all youth engagement groups where assigned youth are present.
  • Serve as team support of programming and behavior management.
  • Observe and interact 1:1 with assigned youth.
  • Participate in the implementation of RIL group programming and services with excellence, adjusting as needed.
  • Ensure programming is engaging, effective and differentiated to needs of each group.
  • Assess program needs and ensure that program objectives are met.
