    Job Title: Home Health Admission Physical Therapist (PT / DPT)

    Company: Home Health Care, Inc.

    Schedule: Full-Time or Per-Diem Availability

    Compensation: $150 / $200 per visit

Home Health Care Incorporated has a fantastic opportunity for a PT to perform admission and start of care visits around the Twin Cities. Our Therapists work in territories – where we have you see clients as close to home as possible If you have a passion for home care, we encourage you to apply

    OASIS PT Responsibilities:

  • Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro
  • Evaluate and assess clients for home health services and create their plan of care
  • Recommend services from other disciplines that have not yet been ordered by the physician
  • Collaborate with clients, families, physicians, and other members of the home care team

    • OASIS PT Qualifications:

  • Licensed as a Physical Therapist in the state of Minnesota
  • Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
  • Prior Home Health Therapy experience
  • Proficiency with Medicare & OASIS documentation
  • Demonstrated ability to work autonomously
  • Excellent at planning, organizing, and communicating
