This is an onsite Stanford Health Care job.

A Brief Overview

This job family is responsible for performing selected delegated functions related to discharge planning, utilization management and patient care coordination.

Communicate and collaborate with the Case Managers, Social Workers and other interdisciplinary team members in coordination of resources and services.

Communicate with review organizations, vendors, providers and other external customers.

Serve as a designated member of the patient care team and work collaboratively under the direction of the Case Managers and Social Workers.

What you will do

  • Assists with compiling resource information and developing resource files.
  • Communicates utilization review, discharge planning and other related information to internal and external customers, including nursing staff, interdisciplinary team members and review organizations.
  • Considers age specific needs of the patient as appropriate and effectively communicates and interacts w/PTS, families, staff and members of the community diverse backgrounds.
  • Documents in the medical record according to established guidelines.
  • Gathers clinical, psychosocial and financial information and communicates with appropriate health care team members, referring patients to professional staff based on established guidelines.
  • Participates on designated patient care teams, contributes to goal setting, process improvement and achievement of desired team outcomes.
  • Performs admission, concurrent and retrospective reviews under the direction of the Case Manager, following established guidelines.
  • Performs clerical activities and data entry related to discharge planning, utilization review, housing, patient care funds, patient disposition, community referrals and quality indicators.
  • Reviews daily admission data and checks inpatient insurance benefits, authorized days and concurrent review requirements.
  • Verifies posthospitalization benefits. Arranges and coordinates transportation, posthospital placement, services and equipment.

Education Qualifications

  • High School Diploma or GED equivalent

Experience Qualifications

  • Three (3) years of progressively responsible and directly related work experience
