    General Purpose of Job:

    The Outreach Health Education Specialist is responsible for the implementation of the Cadena de Madre program and coordination of all program deliverables and any grant requirements.

    The Outreach Health Education Specialist will coordinate, schedule and present the Cadena de Madres educational curriculum in the form of a baby shower.

    The Outreach and Health Education Specialist is responsible for securing shower site locations and monthly scheduling of the program.

    The Outreach and Health Education Specialist will conduct hospital and home visits, and coordinate the participants care with Driscoll Children's Health Plan's Case Managers, Social Workers, and Resource Coordinators.

    The Outreach and Health Education Specialist will make appropriate referrals to other community resources.


    Experience:

    Required: 1 Year in social service to include program administration.

    Preferred: 3 Years in social work or family support service as an educator.


    Education:

    Required:
    High School Diploma or GED.

    Preferred:
    Associates Degree in Social Work or related field.
    Position requires travel within the service area – must have and maintain valid driver license and insurability status. Must pass MVR
