Job Objective:
The Home Health Aide is responsible for providing personal care services according to written directions prepared by the Registered Nurse (RN) and/or Therapist and carried out under the supervision of the appropriate professional.
Duties and Responsibilities:
A. Patient Care
- Follows the Plan of Care established by the RN/Physical Therapist (PT) when providing patient care, and will coordinate with other professional staff when there is a need to revise the aide care plan.
- Provides personal care, rehabilitative and supportive services as assigned by RN or Therapist.
- Make observations for a change in patient status, including skin condition changes for pressure ulcers and pain levels.
- Assists in activities of daily living (personal hygiene, eating, dressing, elimination, exercise and ambulation).
- Provides household duties that are directly related to patient's care.
- Supports the patient and provides care that encourages independence.
- Assists with medications that are ordinarily selfadministered.
- Demonstrates knowledge regarding special needs and behaviors of age specific groups and demonstrates proficiency in required age specific competencies.
- Maintains a safe work environment.
B. Communication
- Ability to communicate effectively while maintaining good working relationships with coworkers, managers and office staff.
- Communicates to the Case Manager changes in the patient condition and needs, and/or those of the family.
- Participates in interdisciplinary case conferences, inservice programs, and committee meetings.
C. Documentation
- Effectively reports and records clinical findings to other interdisciplinary team members, patients, caregivers, and/or representatives.
- Maintains current and accurate clinical records.
- Documentation shows that care was provided according to plan of care.
- Documentation must be accurate and completed timely within agency policy.
D. Education
- Participates in agespecific education and training programs.
Non-Essential Duties:
- Participates in QI process as requested in UR, chart audits, and QI committees.
- Participates as a preceptor for CHHA students.
- Other duties as assigned.
Knowledge, Skills and Abilities:
Knowledge
- Demonstrates that they have completed an approved CHHA training program.
Skills
- Good bedside manner: demonstrates ability to provide good care with compassion and caring.
- Provides excellent direct patient care.
- Provides care based on plan of care.
- Effective oral and written communication skills with case managers/Director of Patient Care Services (DPCS) and patient/caregivers, as appropriate.
Abilities
- Organization, must demonstrate basic ability to maintain schedule of home visits and complete paperwork according to agency procedures.
- Must have good communication skills and have the ability to connect with patients and families in their homes in a kind and compassionate way.
- Ability to project and maintain professionalism in all aspects of patient's care.

Education and Experience:
Education
- Completed an approved CHHA training program.
Experience
- Minimum 1 year experience as a CHHA.
Required Licensure and/or Certifications:
- State certification for Home Health Aide.
- Current CPR certification.
- Maintains a valid California driver's license and evidence of current auto insurance.
