- Master's degree in social work or a closely related field (e.g., Marriage, Child and Family Counseling or Psychology) required.
- Three (3) years of supervised clinical experience.
- Licensed according to California state law (Welfare and Institutions Code section
- Minimum of 2 years of clinical experience with complex care
- Experience with care management plans.
- Knowledge of Social Determinants of Health and Crisis management experience preferred.
- Experience, knowledge, skills, and abilities with culturally diverse populations.
- Speak, read, and/or write another language.

JOB SUMMARY

Under the general supervision supports Program Managers-Managed Care and Program Supervisors-Managed Care by ensuring quality comprehensive care management assessments (including risk, social determinants of health) and plans in managed care programs consistent with the requirements of CalAIM.
