Position Overview:


Partners with Sutter Health patients in unique populations (oncology, bariatric, spinal trauma, stroke, etc.) and guides them through a comprehensive array of health services in accordance with established policies, procedures, protocols, techniques, and guidelines.

Identifies and works to resolve barriers to care, assists with access issues, develops relationships with health care providers, explains insurance and payment systems, and coordinates services with providers, patients, and caregivers within Sutter Health, at external facilities, and in the community to provide seamless and integrated quality care.

Raises awareness about healthcare organizations and agencies information resources, Sutter Health support programs and services, community resources, clinical trials, and helps to meet other illness-related needs identified by the patient, their family, or the patient's support group.

Assists with ongoing evaluation and quality improvement of the program, including community outreach and marketing.

JOB ACCOUNTABILITIES

PATIENT NAVIGATION

  • Provides an ongoing, consistent point of contact for patients and families through the full continuum of their care.
  • Coordinates a plan of care including appointments, education, serving as a liaison throughout Sutter Health and in the community regarding services for this unique population.
  • Offers education, guidance, support, and resource referrals to help patients overcome barriers to their care.
  • Refers patients to resources such as: medication assistance programs, support groups, integrative medicine, genetic counseling, transportation, peer support, financial counseling, home health and/or hospice, relevant organization/agency programs, survivorship programs.
  • Develops navigation forms and patient education materials, including obtaining resource materials for available programs and services.
  • May lead support groups or arrange for speakers that meet the needs of specific populations (e.g., newly diagnosed, posttreatment).

PROGRAM MARKETING AND OUTREACH

  • Develops relationships with referring providers, radiologists, surgeons, medical oncologists, radiation oncologists, hospitalists, and other members of the interdisciplinary team to care for patients with a diagnosis of cancer.
  • Plans and conducts community outreach to raise awareness about Sutter Health's patient navigation services in order to increase the number of patients helped and to better meet the needs of these patients.
  • Maintains current distribution lists, and distributes marketing and outreach materials to patients, volunteers, and clinicians.
  • Develops marketing and outreach materials as needed.

DOCUMENTATION AND DATA COLLECTION

  • Accurately documents patient assessment, care and resources provided, and results of navigation.
  • Records, tracks, and analyzes accurate, complete data needed for statistical reporting, patient surveys, improve navigation programs/protocols.
  • Collects data and documents appropriately in the electronic health record and other computer systems (such as EPIC, laboratory information system (LIS), radiology information system (RIS), MagvVew).

EDUCATION:


  • Equivalent experience will be accepted in lieu of the required degree or diploma._

Bachelor's:
Social Work, Health Education or related field or equivalent education/experience

OR Other:
Graduate of an accredited school of nursing.


CERTIFICATION & LICENSURE:

RN-Registered Nurse of California

OR LCSW-Licensed Clinical Social Worker

OR RCP-Respiratory Care Practitioner

OR PT-Physical Therapist within 1 Year of hire

OR OT-Occupational Therapist

BLS-Basic Life Support Healthcare Provider


Depending on focus area, possess current breast patient navigator certification or attainment of certification within the first calendar year of work.


TYPICAL EXPERIENCE:

5 years of recent relevant experience.


SKILLS AND KNOWLEDGE:


In-depth knowledge of focus area, including diagnosis, current treatment options, complementary therapies, potential complications, family/support group dynamics and associate quality of life issues.

Working knowledge of community resources for the assigned focus area, including requirements and accessibility.

Thorough knowledge of healthcare systems, insurance/payment systems, and medical terminology.

Solid analytical and project management skills, including the ability to analyze problems, reach practical conclusions, and institute effective changes.
