    Full-time, Per Visit - Benefit Eligible

    Schedule:
    40 hours/week

    Per Visit Rates:
    $80,000 - $100,000 estimated annual salary

    Sign-On Bonus:
    $5,000 for full-time

    Supplemental Pay:
    Based on position, schedule, and/or availability: Paid Mileage/Shift Diffs/Stipends

    Mileage and Expense Reimbursement:
    Sixty-two and a half cents per mile – one of the highest in the industry

    Rewarding Work You Will Do:

    • Provide patient care using nursing skills and processes in accordance with applicable scope and standards of practice, following the organization's policies, values, and mission.
    • Responsible for skilled nursing, evaluating client needs, and conducting case management for patients under a licensed physician's direction.
    • Develop and modify an individualized plan of care based on initial and ongoing assessments. Provide skilled nursing coordination of care to patients as determined by the Plan of Care (POC).
    • Collaborate across disciplines and with the provider to coordinate patient care, participate in the plan of care (POC) development, modification, discharge planning, and care conferences as assigned.
    • Educate patients, families, or caregivers regarding resources identified in the plan of care to meet patient goals. Supervise C.N.A.'s and licensed LPN's, delegate specific tasks per Nurse practice act.
    • Participate in related services, such as community education, team training, staff/board committees, and orientation of employees.

    What We Are Going to Love About You:

    • Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
    • Minimum Experience: One to two (1-2) years of experience preferred.
    • Required License: Current Colorado RN license, current CPR certification, valid Colorado state driver license, and proof of auto insurance.

    Physical Requirements: Ability to lift/push/pull a minimum of 50 lbs.

    Colorado Visiting Nurse Association is affiliated with Care Synergy, a nonprofit network of community-based hospice and palliative care organizations serving Colorado's Front Range and the region's Largest Home Health, Hospice, and Palliative Care Network. Care Synergy provides mission support services to better equip Colorado Visiting Nurse Association to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
