- Provide patient care using nursing skills and processes in accordance with applicable scope and standards of practice, following the organization's policies, values, and mission.
- Responsible for skilled nursing, evaluating client needs, and conducting case management for patients under a licensed physician's direction.
- Develop and modify an individualized plan of care based on initial and ongoing assessments. Provide skilled nursing coordination of care to patients as determined by the Plan of Care (POC).
- Collaborate across disciplines and with the provider to coordinate patient care, participate in the plan of care (POC) development, modification, discharge planning, and care conferences as assigned.
- Educate patients, families, or caregivers regarding resources identified in the plan of care to meet patient goals. Supervise C.N.A.'s and licensed LPN's, delegate specific tasks per Nurse practice act.
- Participate in related services, such as community education, team training, staff/board committees, and orientation of employees.
- Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
- Minimum Experience: One to two (1-2) years of experience preferred.
- Required License: Current Colorado RN license, current CPR certification, valid Colorado state driver license, and proof of auto insurance.
