It operates in nine U.S. markets with 50+ senior medical centers. As a fully capitated Medicare Advantage provider, ChenMed offers a high level of care with minimal expense to the patient.

    We are hiring highly qualified Internal Medicine, Family Medicine, Geriatricians, or DO's to join our team in Chicago, IL!

    As a leader in preventive, value-based care, we seek physicians and clinicians who value senior patients, excel in a collaborative environment, and aim for positive patient outcomes.

    Practice Profile

    • Primary care model assuming full risk and accountability for patient outcomes
    • PCPs act as the main coordinators for patient care
    • Support team includes dedicated nurses and case management social workers
    • On-site lab, prescription dispensary, and full scope of in-house specialists
    • Services include patient concierge, transportation, prescription delivery, and holistic health options

    Our Physicians Benefits

    • Manageable patient panel size: 400 patients, with an average of one visit per month
    • Monday to Friday schedule, no nights or weekends
    • Practice call is telephonic only with minimal blocks per year
    • Comprehensive benefits, retirement package, PTO, and CME allowance
    • Three-week orientation program, blackbelt, and leadership training
    • Partnership opportunity after one year
