Job Opportunity: Care Provider for Young Man with Special Needs

Come and join our team to provide exceptional care to a young man in the comfort of his own home. This role is perfect for a nurse seeking a more relaxed work environment with a focus on individualized care.

Hours: 7:30 am to 11 am on Mondays, Thursdays, and Fridays.

Key Responsibilities:

Tracheotomy care and deep suctioning
Use of a cough assist device
Performing a bowel program with stim
Managing suprapubic catheter care
Conducting vital sign assessments
Administering nebulizer treatments
Utilizing a ceiling-suspended Hoyer lift
Assisting with personal care tasks like grooming and dressing
Implementing range of motion exercises
Operating a powered wheelchair
Supporting individuals with spinal cord injuries

Additional Requirements:

Valid driver's license and access to a reliable vehicle
Active licensure
Self-assurance in skills and training
Ability to work independently

Community Horizons provides assistance to self-directed program participants in Wisconsin, offering choices in service providers and delivery methods. While Community Horizons is not the direct employer, our members have the autonomy to make hiring decisions through a third-party fiscal agent overseeing payroll.
