Responsibilities
We are looking for a Referral Specialist to join our Community Health Partners team You will be integral to patient flow and experience.
In this role, you'll be responsible for obtaining referrals and authorizations for the practice and serves as a liaison between the patient/family, physician, treatment team and payor to ensure quality services are being provided and patient needs are met.
Coordinates and tracks all outside appointments, authorizations and referrals as requested by the practice physician(s) for outside diagnostic testing, specialty provider referrals and/or services.
