Case Manager
Hours:
Monday through Friday, 8 am - 5 pm. Will work one Saturday per month.
Position Summary:
The Case Manager is responsible to be primary facilitator of case management services and support for Client Advocate II and III positions, who also provide case management services.
The Case Manager conducts case management assessments, creates individual's case plans and provides case management services in conjunction with community partners utilizing the 6 Principles of Trauma Informed Care.
This includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services.The Case Manager prioritizes relationship over efficiency and demonstrates flexibility in schedule and time to meet the needs of each client.
Duties and Responsibilities:
- Report to Residential Client Services Leader
- Read and update staff log at the beginning and end of each shift
- Read/review client files consistently
- Participate as a member of the staffing team.
- Offers phone support to callers; this includes helping with crisis calls
- Perform case management intake/assessment and crisis support as needed
- Primary case manager and facilitator for high needs cases, such as monolingual, seriously mentally ill, substance dependence, and undocumented clients
- Document all pertinent client information in individual client files
- Provide individualized case management to clients
- Develop and follow through on all individual case management plans and
- Facilitate case management/life skills groups
- Coordinate services with other community agencies
- Maintain files in compliance with local, state and federal requirements
- Follow up on client issues according to Chrysalis program guidelines
- Ability to use Trauma Informed Principles in building relationships with colleagues, clients and community partners.
- Respect and appreciation to all volunteers, donors, and other community members and organizations
- Attend monthly staff meetings
- Prioritize job responsibilities to ensure that appropriate action is taken in critical situations
- Maintain current CPR, First Aid Certification, and TB testing
- To perform additional tasks as requested by management
Knowledge, Ability, Experience Requirements:
- Bachelors Degree in Human Services or related field or equivalent education and experience
- Experience and/or knowledge of the dynamics of domestic violence and the impact on children and families
- Ability to perform crisis intervention
- Experience and/or knowledge of individual and group case management preferred
- Have an understanding of strengthbased model of case management
- Ability to communicate well verbally and in writing
- Ability to utilize effective organizational skills
Other Requirements:
Must be able to pass a fingerprint and background clearance as required by Chrysalis and ADHS licensure requirements; must maintain current CPR and first aid training as required by Chrysalis policies.
Pay:
$ $24.00 per hour
