- Creating and updating personalized care plans for patients.
- Collaborating with patients' support systems to develop treatment plans.
- Identifying and enlisting high-quality healthcare providers.
- Coordinating with healthcare providers to ensure patients receive necessary treatments.
- Educating patients on procedures, instructions, and referrals.
- Connecting patients with social service programs for additional support.
- Conducting regular follow-ups to monitor progress and enhance health outcomes.
- Maintaining detailed records of case management activities.
- A Bachelor's degree in a relevant field such as social sciences, nursing, or social work.
- At least 2 years of experience in public health or a related field.
- Current case management experience of at least 1 year.
- Preferably hold a Certified Case Manager (CCM) license as an RN, LPN, LSW, or LCSW.
- Proficiency in Healthcare Management systems and MS Office, especially Excel.
- Strong problem-solving skills and the ability to navigate complex care systems.
- Excellent communication skills and the capacity to build positive relationships with patients and healthcare team members.
- The flexibility to work both independently and collaboratively within a team.
- Willingness to work occasional weekends as needed.
