Job Title: Community RNDepartment: Community Living

Reports to: Assistant Executive Director

Job Summary: The Community RN is an essential position that provides oversight of the health and safety of the individuals served in the Nodaway County Service's Community Living Division.

Supervisory Responsibilities Not applicable

Duties/Responsibilities:

Monitors the health and safety of the individuals receiving residential services
Identifies nursing supports individualized to specific consumer needs, including specified delegations
Completes monthly face-to-face assessments of the individuals and records findings
Reviews the individuals' records monthly including physicians' orders and medication administration records (MARS)
Tracks doctor's appointments and assists at appointments as needed. Tracks trending and current labs, updating systems, staff, and MARS with all medication changes
Maintains communication with community living program managers; including reporting and documenting all changes in the individual's health status, needs, and identified deficiencies in the standard of care provided to individuals
Collaborates with Community Living Program Managers about processes, procedures and trainings related to the ongoing care of individuals supported
Facilitates Level One Medication Aide training and recertifications utilizing DMH approved curriculum
Assists, monitors and tracks all staff delegations for renewals and accuracy in regard to individuals' needs
Follow all required regulations required by DMH, Licensure & Certification, and other monitoring entities
Collaborate and help write/implement new policies as needed by NCS
Provides allocated nursing units monthly as authorized
Ensures the Monthly Service Log is completed accurately each month and submitted for billing
Other duties as assigned

Excellent time management skills with a proven ability to meet deadlines
Ability to prioritize tasks
A self-starter

Education and Experience

Qualifications:

Current Missouri Licensed RN
First Aid/CPR in good standing and eligible for position
High school diploma or equivalent
Ability to drive and maintain a valid driver's license

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer
Must be able to lift up to 50 pounds at times
